Change Your Address/Name

Change Address - Two Options

1. Online: Log in to your account and choose Account Management option on the left, then click on Address Info.

or

2. Print, Fax, Mail: Complete this Address Change Form  and return it to the Records and Registration Office

Change Name - Hard Copy Only
